You must file an insurance claim directly to your insurance company for reimbursement.

To have information transferred to a Georgia 3300 form, parents need to bring a copy of physical exam done in the last year, prior to school enrollment with hearing, vision and dental screenings clearly marked.

- Urine for a pregnancy test
- Finger stick for lab
Medicaid application (one of the following documentation is needed:

- USA Passport or Naturalization Certificate, or I94/Green Card)
- WIC referral

Prenatal Case Management
- To improve birth outcomes by assisting pregnant Medicaid eligible women in gaining access to needed medical nutritional, social, educational other services.

Breast and Cervical Cancer Program
- The Breast and Cervical Cancer Program was established to provide screening and follow-up services to low income, uninsured/under-insured women throughout the state of Georgia because Early Detection Saves Lives.

- Women 40-49 With a family history of breast cancer (mother, sister, aunt) experiencing problems
